WHAT TO BRING

​If you are a first-time patient, please arrive early and bring the following items to your appointment:

• Insurance card(s)​
• Picture ID
• List of all prescription and over-the-counter medications that you are currently taking
• Any required co-payment

Please give our office 24 hours notice if you are unable to keep your appointment, as we do charge a $25.00 no show fee for missed appointments.
